When the Change Happened

Stage 1: Optimising the Foundation

  • Introduced new CDM and Health Assessment templates, tailored for efficient, compliant care
  • Supported the new Practice Manager with structured training and weekly guidance, building her confidence and control over CDM workflows
  • Increased MyMedicare registrations, supporting long-term patient engagement and eligibility for future incentives
  • Care Plan appointments were reduced from 45 minutes to 30 minutes, and are now on track to reach a 20-minute delivery standard

Stage 2: Scaling Performance

  • Weekly CDM and Health Assessment output grew from 10 to over 50, now representing 15% of total billings
  • The CDM process was fully streamlined and standardised across the team
  • Nurses became central to workflow execution, delivering faster, repeatable, and MBS-aligned care
  • Introduced new nurse-led services including Iron Infusion Clinics, Body Scan Clinics, and additional chronic condition support programs
